Some insurance companies require an appointment date before they give a prior Authorization. If this is the
situation call Barbara at 314-623-7575 to make the appointment first
Call the mental health # on the back of your Insurance card and obtain following Info:
1. Is Barbara a provider for your insurance company? A. If not does your plan have out of network benefits?
2 Do you need a prior authorization for your sessions?
A. If yes get authorization #, the # of sessions authorized, beginning & ending date of the authorization and
bring this info with you to the 1st session
B. If no prior authorization is needed ask how many session you have per year
3. Also ask if you have a co pay and/or deductible and how much each of those are

grocery store on left, soon you will see Imo's sign up high. Just past sign is driveway to turn in.
After turning into driveway looking straight ahead you will see Imo's and I am the 2nd door on
right with white sign with dark letters with my name on it.
From Hway 21 and Hwy 110 Go North on Hwy 21. Go past B Hwy on left (light) on right see
Imo's sign up high. Drive way to turn in is right before sign. I am 2nd door on right from Imo's.
There is a white sign in window with my name.
From North 141 & 21 South on New Hwy 21 stay on New 21 Hwy until B Hwy. Left on B Hwy
left at old 21. Less than 1 block see Imo's sign up high on right. Turn in driveway before sign. I
am 2nd door on right of Imo's. There is a white sign in window with my name.
