BEFORE YOU ATTEND COUNSELING CALL YOUR INSURANCE COMPANY AND ASK THE
FOLLOWING QUESTIONS:
1. Is Barbara a provider for your insurance company? A. If not does your plan have out of network benefits?
2 Does your insurance plan require you to get a prior authorization for counseling?
A. If yes get authorization #, the # of sessions authorized, beginning & ending date of the authorization and bring this info
with you to the 1st session
B. If no prior authorization is needed ask how many session you have per year
3. Also ask if you have a co pay and/or deductible and how much each of those are
PRINT AND COMPLETE THE CL INTAKE FORM and CLIENT HISTORY FORM and bring
them along with your insurance card with you to the 1st session
When the appointment is for a small child the child is not to be present during the intake
ALL CO PAYS & DEDUCTIBLES ARE DUE AT TIME OF SERVICE

From Old Hway 21 & A Hwy go So. go past Hardee's on left, Court house on right, Queens grocery store on
left, soon you will see a BP Service station on left, then on right is my office. There is a "Welcome Home
Realty" sign on the porch of the building. I am on the South end. There is a sign in the window with my
name.
From Hway 21 and Hwy 110 Go North on Hwy 21. Go past B Hwy on left (light) just slightly less than 4
tents of a mile on left side is my office. There is a "Welcome Home Realty" sign on porch of the building
and I am in the South end. There is a white sign in window with my name.
From North 141 & 21 South on New Hwy 21 stay on New 21 Hwy until it ends at B Hwy. Left on B Hwy
and left on Business Hwy 21 (light). Slightly less than 4 tents of a mile on left side is my office. The
building has a "Welcome Home Realty" sign on porch and I am in the South end. There is a white sign in
window with my name.
